UnitedHealth Group Hospice Nurse Practitioner - Las Vegas, NV in Las Vegas, Nevada

We are currently seeking a Hospice Nurse Practitioner to join our team in Las Vegas.

In this position you will provide nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. You will work under the direction of the Hospice Medical Director. You will join and assist in providing comprehensive medical care and provide direct care to hospice patients in their homes and/or skilled nursing facilities. You will work with other caring, friendly, healthcare professionals and the Medical Director to determine the best overall care for your terminal patients (prognosis of 6 months or less). You will provide emotional support for the patient and the families in your care on a personal level. You will provide education on Hospice and Palliative Care for physicians, patients and families.

Primary Responsibilities:

  • Perform face-to-face evaluations.

  • Make visits to patients at home and skilled nursingfacilities as needed

  • Assist with Palliative Care consults when needed

  • Rapidly formulate a differential diagnosis fromassessments conducted within the clinical setting

  • Develop and implement treatment plans for patientsassigned within the clinical setting

  • Develop and actualize plans for patients as assigned

  • Provide guidance to nursing personnel towards treatmentplans and care issues

  • Document treatment plans in the medical record

  • Document collaborative guidance from the physician asneeded

  • Coordinate care with the patient and family/caregivers

  • Basic technical proficiency in the use of computerprograms for medical records or internet search for clinical practice

  • Ability to communicate with patients, families, primarycare team members and management staff about terminal care and advancedillness

  • Ability to make care decisions based on best medicalpractices in specialty area of practice without direct supervision orguidance by a physician

  • Ability to formulate a differential diagnosis

  • Knowledge of hospice regulations and working withinthem

  • Proficient in assessment and development of treatmentplans

  • Ability to conceptualize the needs for the team ordepartment and actualize plans for implementation

  • Empowered to develop plans of action and gain team consensustowards common goals

  • Ability to work independently

Required Qualifications:

  • Master of Science degree in Nursing

  • Active, unrestricted APRN license in Nevada orability to obtain

  • DEA license or ability to obtain

  • National Nurse Practitioner certification in PalliativeCare, Family Medicine, Adult Medicine or Gerontology or ability to obtain

  • 1+ yrs. of NP experience (preferred)

  • Youwill be provisioned with appropriate Personal Protective Equipment (PPE)and are required to perform this role with patients and members on site,as this is an essential function of this role

  • Employeesare required to screen for symptoms using the ProtectWell mobile app,Interactive Voice Response (i.e., entering your symptoms via phone system)or a similar UnitedHealth Group-approved symptom screener prior toentering the work site each day, in order to keep our work sites safe.Employees must comply with any state and local masking orders. Inaddition, when in a UnitedHealth Group building, employees are expected towear a mask in areas where physical distancing cannot be attained.

Preferred Qualifications:

  • Work experience in hospice, palliative care, acutecare, oncology, ambulatory care and/or pain management

  • Knowledge of end of life care, including advanced careplanning, pain and symptom control of the terminally ill and bereavementcare issues are desired
