Investigates, assesses and plans therapeutic interventions and support to help patients and their family members cope with social, emotional, economic and environmental problems.
Assesses patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
Assesses patient and family's financial, social, emotional, educational and environmental resources and identifies needs based on patient's current medical condition.
Develops an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patients medical condition.
Participates in Discharge planning activities for complex patients in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
Other duties as assigned.
Bachelor's Degree in Social Work from a CSWE approved school of Social Work.
Possesses knowledge of and practices in accordance with the NASW Code of Ethics.
Licensed Social Worker (LSW) in the current State.
Social Worker or Case Management Certification (CCM) within one year of eligibility.
Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire.

Requires full range of motion, manual and finger dexterity and eye-hand coordination.
Requires standing and walking for extensive periods of time.
Requires normal or corrected hearing and vision to normal range.
Sedentary work exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3 of the time) and /or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.
